Overview
The Destinations tab on the Physician Group Analyze page shows where the selected physician group's home health patients are going — which home health agencies are admitting patients treated by the selected group, and how those patterns have trended over time. This is the tab that turns group-level volume data into actionable leakage intelligence.

This tab is the competitive picture — it shows which home health agencies the selected group's patients are going to, and how concentrated that referral flow is. To get here: open the Physician Groups Explore page, click a group's name to open its Analyze page, then click the Destinations tab.

FFS Home Health Destinations
The FFS Home Health Destinations table shows the home health agencies that received Medicare FFS patients from the selected physician group during the reporting period. Each row represents one home health agency. Sort by HHA Patients (Overall) to rank destinations by volume, or sort by HHA Affiliation % to see which agency holds the highest percentage of this group's home health patients.
Claim Type Dropdown
At the top left of the table there is a dropdown that allows you to choose the claim type from which the metrics in the table are calculated.
This dropdown allows you to choose which patient type you want to focus on.

Understanding the Metrics

| HHA Name | The name of the home health agency as listed in the NPI Registry. Click the name to open the HHA Analyze page for that agency. |
| NPI | The National Provider Identifier for the listed home health agency. |

| HHA Patients (Overall) |
The count of distinct Medicare FFS home health patients associated with this physician group who were admitted to the listed home health agency during the one-year reporting period.

| HHA Affiliation % | The percentage of the selected physician group's total home health patients who were admitted to the listed home health agency during the one-year reporting period. |
| HHA Patients: Followed | The count of distinct Medicare FFS patients who received home health services from the listed HHA for whom a physician in the physician group was listed as the certifying physician on the home health claim during the one-year reporting period. |
| HHA Patients: 3 Months Prior | The count of patients admitted to the listed home health agency for whom a physician in this group billed a Medicare claim in the three months prior to the patient's home health admission. |

Destinations/Sources Preview
When you click on the magnifying glass icon
to the left of a specific home health agency's name, the Destinations/Sources preview panel for that home health agency opens. See image below.
This panel shows the top five facility and physician sources for the selected home health agency along with metrics for each source.
Despite the mention of "Destinations" in the panel name, there are no destinations listed in the panel.

Putting the Data to Work
Find your agency in the table — and check your affiliation percentage. Sort by HHA Patients (Overall) to rank destinations by volume. Locate your agency and check your HHA Affiliation %. That number is your agency's percentage of this group's home health patients. The gap between your percentage and the top agency's percentage is the size of the opportunity.
Identify your most direct competitors. The agencies above you in the table are the ones this group currently prefers. Review their Readmission Rate and Hospitalization Rate — if your agency's clinical outcomes are stronger, you have a data-backed case to make in your next conversation with the group.
Use the quarterly trend to time your outreach. The HHA (Overall) Distinct Patients quarterly columns show whether a competitor agency's share is growing or shrinking. A competitor whose quarterly counts are declining is losing ground — that's the moment to step in with a compelling case for your agency.
Check the 3 Months Prior count to understand referral timing. A high HHA Patients: 3 Months Prior count to a competitor means that group's physicians are actively treating patients shortly before their home health admissions go elsewhere. Those are the physicians — visible on the Physicians tab — who most need a direct relationship with your agency.
